The video is by Tex Grebner who explains how he had a ND. He was using 5.11 thumb drive holster with his Glock earlier in the day, then switched to his Kimber 1911 with Blackhawk Serpa holster.
When he tried to do a drill, he disengaged the safety on the pistol, and after he pressed the holster's retention system with his index finger, he curled up his finger and pulled the trigger.
